Schaumburg Township Council of PTAs Elementary Summer Scholarship

2008 APPLICATION FOR ELEMENTARY SUMMER SCHOLARSHIP

As part of its commitment to life-long learning, the Schaumburg Township Council of PTAs (STC) offers scholarships for summer programs/camps to District 54 students currently in 4th, 5th, 6th, and 7th grades who wish to broaden their education, talents and experiences.

A full and complete application must include all the following:

1. the Identification and Certification Form
2. the Student Statement Form consisting of two (2) separate questions with a limit of 200 words
3. the Parent Statement Form with a limit of 200 words
4. a copy of the program or camp brochure and application (including cost information) 
5. a copy (not the original) or membership chairman written verification of the parent/guardian PTA/PTSA membership card for the 2007-2008 academic year before November 1, 2007. 

Please note that any application not meeting all the criteria will be disqualified.
No materials will be returned.

The student and the parent/guardian statements must be on the STC application form and fit in the space provided, either typewritten, computer generated or neatly handwritten with text able to be clearly copied.  Each response must be 200 words or less.  There must be a separate application submitted for each student for each program/camp and for each session of a camp/program.  No submitted materials will be returned.

A limited number of scholarships for up to 50% of program or camp tuition will be awarded.  The individual amount awarded will not exceed $200.00.  All scholarship checks will be forwarded directly to the program agency or the camp.  Should a recipient not attend the camp/program for which a scholarship was awarded, it is the responsibility of the recipient's family to reimburse STC. 

Students may apply for as many scholarships as they choose.  Each session of a camp/program must have a separate application.  No household will be awarded more than one (1) scholarship.  Any one (1) household may be awarded a scholarship for only two (2) consecutive years.

Please return a completed application in a sealed envelope to the school office.  The deadline for submitting an application is April 11, 2008, or as designated by the Elementary Scholarship Coordinator of your PTA/PTSA.  All candidates will be notified of committee decisions shortly after May 1, 2008.

Scholarship recipients are encouraged to notify STC describing the benefits of having attended the camp/program for which the scholarship is received.
